Protecting your personal information is important to us. So is following New Zealand’s privacy laws. This Privacy policy tells you how we will protect your privacy and collect and use your personal information (any reference to “we”, “us” or “our” is to Your Health NZ). “Personal Information” means any information that can directly or indirectly identify you or another person. We will manage your personal information in accordance with the terms of this Policy.

YOUR RIGHTS

If you don’t agree with our Privacy Policy or don’t want us to collect your Personal Information, then you shouldn’t use our services. If you change your mind and don’t agree with our Privacy Policy anymore, then you should stop using our services. If you have any questions, you can contact our Privacy Officer at info@yourhealthnz.co.nz. We’ll only use your Personal Information for the reasons that we collected it. If we don’t need your Personal Information anymore for those reasons, then we’ll stop using it.

Providing some information is optional. However, If you choose not to provide relevant details as requested in regards to informed consent, we’ll be unable to provide vaccination and/or healthcare services to you.

WHY WE USE AND SHARE YOUR INFORMATION

  • To respond to your questions and to tell you more about us
  • For our internal records
  • To comply with our professional and legal obligations
  • For personal safety or that of the public
  • To provide our business services
  • Assess, inform and provide health care and/or health & wellbeing advice
  • For marketing purposes
  • For any other purposes that you authorise

Besides our staff, we may share this information with:

  • Our contractors or agents
  • One or more of the National Immunisation databases, in accordance with the Ministry of Health and /or Public Health requirements
  • General Practitioner in order to inform them that our client/patient has either received nursing care or a vaccination (if GP is accessible and if notification is required).
  • Centre for Adverse Reaction (CARM) in order to provide feedback on any side effects or reactions to medicines (only if applicable)
  • Next-of Kin in order to notify them of any urgent or emergency situations involving our client/patient
  • Green Cross Pharmacies: Names only with regards to Voucher redemption (only if relevant and necessary)
  • Emergency services (in the event we need to access their services)
  • Governmental agencies, law enforcement agencies, regulatory body and others, if required to do so by law, court order, or other legal process or where we are requested to do so by an authorised governmental or law enforcement authority
  • Governmental, law enforcement agencies, regulatory body, medical agencies, others and/or employer/s (only if we are engaged with you in your workplace); if we believe in good faith that disclosure is desirable to lessen or prevent a serious threat to an individual’s life, serious threat to health or safety, or a serious threat to public health or public safety

HOW WE PROTECT YOUR INFORMATION

We keep your information safe by:

  • Using password protected devices
  • Securing paper copies in lockable storage and only allowing certain staff to access it
  • Our staff, and other health professionals have a professional responsibility to protect
    client confidentiality

Security safeguards as are reasonable in the circumstances to take, against:

  • Loss
  • Access, use or modification
  • Other misuse

We keep your information for ten years at which point we securely destroy the information

SEEING OR CORRECTING YOUR INFORMATION

Under the Privacy Act 2020, You have the right to ask for a copy of any personal information we hold about you, and to ask for it to be corrected if you think it is wrong. If you’d like to ask for a copy of your information, or to have it corrected, please contact us at info@yourhealthnz.co.nz Where applicable, we may charge a fee for responding to a request for access to and/or disclosure of personal information.

PRIVACY COMPLAINTS

If you think that we haven’t properly protected your privacy, you can tell our Privacy Officer here (info@yourhealthnz.co.nz), or complain to the New Zealand Privacy Commissioner. All complaints will be managed in accordance with our Complaints Policy.

PRIVACY POLICY CHANGES

If we change our Privacy Policy, then we will tell you about it on our Website. The changes will apply 14 days after we post them. If you don’t agree with the changes, you should stop using our services. If you keep using our services, then this will indicate that you have agreed to our changes.
